*The following insurance carriers are exceptions to this service: Unicare, MDIPA/Optimum Choice, Aetna HMO, Medicaid (through Amerigroup and Healthkeepers), and United Healthcare. These providers only allow the referring physician's office to obtain the pre-certification/authorization.
